Sandbar Health is a physician-led concierge primary care practice with its own health plan attached. We deliver the care and we carry the cost of it, which means our incentives sit with our members' health instead of with the number of visits on a given day.
We are hiring our founding Behavioral Health Clinician. You would be the first behavioral health hire in the practice, providing therapy and short-term behavioral health care embedded directly in the care pod, alongside the physicians, APPs, and care team.
What you would do
- Provide integrated, embedded behavioral health care inside the care pod, working closely with physicians and APPs through warm handoffs and shared care plans
- Conduct behavioral health screening (PHQ-9, GAD-7) and assessment, and deliver evidence-based therapy and brief interventions
- Carry a therapy caseload across in-person, virtual, and asynchronous visits
- Set clear escalation and referral pathways for patients who need a higher level of behavioral health care
- Reach out proactively to patients with behavioral health needs, care gaps, or elevated risk
- Work alongside our care navigation team, who own logistics and system navigation, so patients are not left to coordinate their own care
- Help shape our behavioral health pathways and standards, and flag when the evidence or the reality on the ground says to change them
The practice
We built the practice around small panels and real access. Each pod is one physician, one medical assistant, and up to four APPs, with health coaching and behavioral health support built in. Panels run 500 to 750 patients per provider, to enable more time spent with each patient.
A few specifics clinicians usually want up front:
- Panel: 500 to 750 patients per provider across the pod
- Pod staffing: 1 MA per physician, up to 4 APPs, plus health coaching and behavioral health
- Access: same-day and next-day availability across in-person, virtual, and asynchronous visits
- After-hours and call: partner-led telemedicine platform
Because panels are small, visit volume runs lower than a standard practice. Sessions run longer, and low-acuity needs get handled asynchronously when that serves the member better than a trip to the office.
Compensation and benefits
- Base salary of $95,000–$130,000, with bonus potential

What we're looking for
Required
- Licensed clinical social worker (LCSW) or licensed clinical psychologist (PhD or PsyD)
- Active or eligible licensure in DC, Maryland, and/or Virginia, with willingness to license in additional states to support remote patients
- Training in brief, evidence-based interventions (CBT, motivational interviewing) and short-term therapy
Preferred
- Experience delivering integrated or collaborative behavioral health in a primary care or value-based setting
- Comfort with multi-state licensure and telehealth-based care
- Someone who likes team-based care and is excited by the ambiguity of building something early
