Sandbar Health is a physician-led concierge primary care practice with its own health plan attached. We deliver the care and we carry the cost of it, which means our incentives sit with our members' health instead of with the number of visits on a given day.
We are hiring a Family Medicine Physician to join our founding care team. You would practice full-scope Family Medicine inside a pod built to give you time with your patients, working alongside APPs, medical assistants, health coaches, and embedded behavioral health.
What you would do
- Practice full-scope outpatient Family Medicine for adult and pediatric patients across the care journey, in person and virtually
- Work as part of a pod alongside APPs, medical assistants, health coaches, and embedded behavioral health
- Run preventive care and chronic disease management with an eye on keeping people healthy and out of the hospital
- Perform office-based procedures and women's health services within your scope
- Help shape our clinical pathways and standards, and flag when the evidence or the reality on the ground says to change them
- Work closely with our care navigation team, so patients are not left to coordinate their own care
The practice
We built the practice around small panels and real access. Each pod is one physician, one medical assistant, and up to four APPs, with health coaching and behavioral health support built in. Panels run 500 to 750 patients per provider, to enable more time spent with each patient.
A few specifics physicians usually want up front:
- Panel: 500 to 750 patients per provider
- Pod staffing: 1 MA per physician, up to 4 APPs, plus health coaching and behavioral health
- EMR: Canvas, with ambient AI note-taking, so your charting does not follow you home as pajama time
- Access: same-day and next-day availability across in-person, virtual, and asynchronous visits
- After-hours and call: partner-led telemedicine platform
Because panels are small, visit volume runs lower than a standard primary care day. Appointments are longer, and low-acuity needs get handled asynchronously when that serves the member better than a trip to the office.

What we're looking for
Required
- Board Certified in Family Medicine
- Active or eligible medical licensure in DC, Maryland, and/or Virginia
- Comfortable caring for patients across the care journey, including preventive care, women's health, and common musculoskeletal complaints
Preferred
- A background in concierge, value-based, or lifestyle medicine. Urgent care, emergency medicine, and rural practice translate well too
- Comfort with digital-first patient communication
- Someone who likes team-based care and is excited by the ambiguity of building something early
